Care coordination is available for chronic diseases of lifestyle such as HIV, Diabetes, Asthma, COPD, Hypertension, Cardiac Failure, HIV, Depression, Bipolar disorder and Substance Abuse, aimed at reducing “lost to follow up” rates of high-risk individuals as well as absenteeism rates.
